Consent to Use of Registration Data and Likeness

By filling out the registration form for the 2024 Dysferlin Conference (the “conference”), I expressly consent to the Jain Foundation processing the data provided by me or on my behalf to manage my registration and participation in connection with the conference. In addition, I agree to have my name, institution, city, state, email address, and picture included in the list of participants in the conference booklet and on the Jain Foundation website so that other conference participants will be able to contact me on an individual basis. In addition, I allow the Jain Foundation to take videos and photographs of me at the conference and grant the Jain Foundation permission and consent to use my photographs, videos, name, and likeness without restriction for any lawful marketing, publicity, promotional or organizational purpose, including on its website. I expressly release the Jain Foundation, its representatives and employees, from any claims arising from such use or distribution of the foregoing, and I waive any right to inspect or approve any finished products or copies of printed matter that may be used with the foregoing.

Assumption of Risk; Indemnification

By registering for the conference, I hereby assume all risks associated with, resulting from, or arising in connection with, participation, in any capacity, in the conference or any conference-related activities, including, without limitation, all risks of theft, loss, harm, damage, illness or sickness (including those caused by known or unknown infectious or communicable disease such as COVID, H1N1, or other illness or sickness caused by bacteria, fungi, microorganisms or viruses), or injury to the person (including death) or property damage, whether caused by negligence, intentional act, accident, act of God, or otherwise.

As a condition of my participation in the conference, I agree to hold harmless, defend and indemnify the Jain Foundation, its directors, officers, employees, their representatives, heirs, executors, administrators, successors and assigns (the “Protected Parties”) from any and all claims of mine, my spouse, family members, or others arising from my injury or loss due to my participation in the conference, including those arising from the inherent risks of attending the conference or the negligence of the Protected Parties to the greatest extent allowed by law.

Confidentiality

To encourage open communication, each attendee of the conference agrees that any information presented at the conference, whether in a formal talk, poster session, or discussion, is a private communication from the individual making the contribution and is presented with the restriction that such information is not for public use. Prior to quoting or publishing any such information presented at the conference in any publication, written or electronic, written approval of the contributing attendee must first be obtained. The audio or video recording of lectures by any means, the photography of slide or poster material, and printed or electronic quotes from papers, presentations and discussion at the conference without written consent of the contributing attendee is prohibited. Scientific publications are not to be prepared as emanating from the conference. Authors are requested to omit references to the conference in any publication, written or electronic. These restrictions apply to each attendee of the conference and are intended to cover social networks, blogs, tweets or any other publication, distribution, communication or sharing of information presented or discussed at the conference. Each attendee of the conference acknowledges and agrees to these restrictions when registration is accepted and as a condition of being permitted to attend the conference.
